Augmentation rhinoplasty with diced rib cartilages

Augmentation rhinoplasty is one of the most common aesthetic procedures in Asian countries.

Because commonly a large volume is required for augmentation, silicone implants are widely used in Asia.

However there are people who want to have augmentation rhinoplasty using autologous rib cartilages. 

 

- Some people have abhorrence of silicone, 

- Some people have had bad experience of complications from silicone implants,

- Some people have anatomic deficiencies, such as poorly projected tip and saddle nose deformity, which must be reconstructed with rib cartilages. 

 

In those cases, rib cartilages are used for augmentation rhinoplasty.

In the past, carved rib as one piece was used. 

However, because of the problems related to rib such as warping, visible contour, recently newer methods utilizing diced ribs have been developed. 

 

One of them is DCIF (diced cartilages in fascia). In DCIF, diced ribs are wrapped in fascia. 

The shape of DCIF is like a sac, and it is not very aesthetic. The procedure is complex and it is difficult to tightly pack the diced cartilages in a sac of a fascia.

 

Another is DCGG (diced cartilages glued graft), in which diced cartilages are glued with commercial or autologous glues.

Commercial glue is not completely free from the virus or prion transmission. 

Autologous glue is free from disease transmission, and there are some growth factors in it enhancing tissue healing and regeneration. However, autologous glue is weaker in bonding strength compared with commercial ones.

 

Below are the photos of a patient, who had augmentation rhinoplasty using diced rig cartilages which were glued with autologous fibrin glue, and tip projection wad made with rib cartilages also.
